Credit Card Payments

If you are paying by credit card when mailing or faxing in an application, the Division of Corporations, Business and Professional Licensing requires that you submit the payment information on the credit card payment form provided by the Division.

The credit card payment form must be filled out completely, including the fee and signature areas. The Division cannot accept credit card information submitted in any other format.

This interactive form allows you to type in the required information and then print and sign it. The form and any supporting documents may be sent to the Division either by mail or fax. Do not email the credit card payment form or credit card information.
