If you are paying by credit card when mailing or faxing in an application, the Division of Corporations, Business and Professional Licensing requires that you submit the payment information on the credit card payment form provided by the Division.
The credit card payment form must be filled out completely, including the fee and signature areas. The Division cannot accept credit card information submitted in any other format. The online form is "Interactive", in which you may type all the information, print and sign the form, and then mail or fax the form with the required documents.
